Stages of Fixed Braces

STAGES OF FIXED BRACES It is an important decision to go through orthodontic treatment. One main reason is the relatively long duration of this treatment. Comprehensive orthodontic treatment duration changes significantly depending on the malocclusion. We may say that it takes between 6 months to 30 months in general. Of course there are special cases where it may take even longer. Mainly there are 3 stages for a full orthodontic treatment Stage 1 – Planning Your orthodontist makes an evaluation of your dental health supported with the information you provide about your general health. X-rays are crucial during primary evaluation to assess unseen cavities as well as surrounding bone structures. Orthodontic models and orthodontic photographs are also taken for this purpose. Types of braces, necessary extractions and additional devices that will be used is decided at this stage. The need for retention (Stage 3)  is also discussed with you at this stage.  Stage 2 – Active Treatment Depending on your choice of braces, brackets are temporarily glued to your teeth (Bonding). A type of composite resin, which is used for dental fillings, is used. An initial cleaning may be performed too. Normally orthodontic treatment starts with flexible low diameter wires and progresses towards stainless steel rectangular wires. After bonding of the brackets, for the first week, you may experience some pain. This is completely normal. Pain is more noticeable when you bite on something. Therefore you may take pain killer 20 mins before eating if you are having difficulty.  After first week, everything will be back to normal. If you are having lingual braces you may have difficulty with pronunciation. Depending on the type of lingual braces, the time that you need to get used to braces changes greatly. At this stage your teeth are moved to their ideal if not, optimal position as planned in the first stage. Once your orthodontist and you are happy with the final position of your teeth, next step is debonding. Detaching the brackets from the teeth surface is fairly a simple procedure. Aesthetic brackets may be a little more difficult to detach from teeth. Afterwards, remaining composite resin material is removed from teeth surfaces. Stage 3 – Retention Unfortunately after debonding, if we leave the teeth free, they start to move back to their original position. First 2 years are very critical. However, even after that period, orthodontics literature shows us that there is a chance that your teeth may move to a certain extent. In some malocclusions there is a less chance and in some others it is almost inevitable. Therefore a form of stabilization is necessary. This is called retention. It may be temporary or permanent. For permanent retention generally a lingual retainer is bonded behind the teeth. A fine retention wire is used for this purpose. This wire does not interfere with your speech. It is not visible to others and it is very easy to get used to.For removable retention, Hawley or ESSIX retainer are main options.  Duration of retention period is discussed with you in the planning stage of your orthodontic treatment.

Is There a Age Limit for Braces?

IS THERE AN AGE LIMIT FOR BRACES? In todays world, with the constant progress in medical technologies, it is easier to provide a more comfortable orthodontic treatment experience for adult patients. The percentage of adult patients among the total patient group is significantly higher compared to 20 years ago. Among many others, economical, social or medical, one main reason for this increase is the new methods that we offer to patients and unfortunately, the need for a treatment repeat that wa performed when patient was at a young age. There is no age limit to straighten your teeth and improve your smile. This does not mean that the treatment applied to a 12 year old will also apply to an adult. Main difference between an adult and a child is the growth potential. In some cases orthodontist needs to use this growth potential to manipulate the skeletal growth. In cases where the jaws are narrow, she/he uses this potential to expand the jaw width. If the bottom jaw growth is not catching with the top one, she/he uses this potential to influence the growth of the bottom jaw. If there is a deep bite or open bite, which front teeth do not meet when patient is biting, again growth potential is needed to influence skeletal growth.  In an adult patient, if there is a significant discrepancy between the bottom and top jaw in any planes of the space, orthodontics alone may not be sufficient. In this is the case, a combination of orthodontics and surgery would be necessary. Luckily for most of the patients this is not the case.  For a wide range of cases with skeletal discrepancy, a camouflage treatment is applied. Never think that ‘I am too old for braces’; once a patient completes skeletal growth, the treatment for a 20 year old and for a 60 year old person is not different if the teeth and surrounding structures are healthy. You are never late for getting a natural beautiful smile.

TOOTH COLOR/CLEAR BRACES

TOOTH COLOR/CLEAR BRACES The leading reason for demanding orthodontic treatment is aesthetics. However the treatment itself may not be that aesthetic for some patients. If you would like to have orthodontic treatment and concerned about the appearance of metal braces, this type of tooth colored ceramic or clear braces are ideal for you. You will have all the advantages of braces over clear aligners with considerably less visibility compared to metal braces.  Aesthetic brackets The brackets under this group are called “aesthetic” not because they are aesthetically pleasing but rather less visible compared to metal brackets. These are the choice for most of the adult patients. These brackets are made from a range of different materials. Today the most popular ones are monocrystalline and polycrystalline sapphire brackets. There are also composite resin and zirconia brackets. While less visibility is the top advantage of these braces, they have some disadvantages that your orthodontist can shed some light on these.  In orthodontics teeth slide along the archwire. Brackets are the elements between the teeth and the wire. Less frictional force means more teeth movement. Metal to metal friction tends to be smaller than metal to ceramic friction. This means with classical ceramic braces teeth movement would be more difficult. Today, some brands of cristalline and zirconia brackets are as efficient as metal ones in terms of friction. The other disadvatage is that they are more brittle compared to metal ones. This is a disadvantage when applying force to brackets through archwire. Eventhough they may have same disadvantages over metal ones you may choose this type of braces without hesitation. Aesthetic braces tend to be more expensive than metal braces. If you would like to have a relatively faster treatment with these braces, it should be one of the best brands in this field. This means unfortunately a little more expensive treatment.
